The following forms are required for participation by all Juniors and Masters Team rowers. Please read, complete, and return these forms to your coach (or email to the Executive Athletics Director, Lisa Glenn) as soon as possible. The forms will only need to be completed once for your time with Atomic Rowing, unless noted otherwise.
